A hospitalist is a physician who is a specialist in caring for you when you’re hospitalized. Your primary care provider may request that a hospitalist be in charge of your care during your hospital stay. This way, you have the benefit of being seen by a physician whose practice is entirely focused on the care of hospitalized patients.
Coordinated Care Health Center
Transitioning from hospital stay to home can be difficult because there is a lot of new information. A special team is formed to care for patients once they leave the hospital and transition back to home care.
Who is on the Team?
Physician – your doctor is your coordinator of care and works with the team to make certain you’re getting the best care possible
Registered Nurse (RN) or Licensed Vocational Nurse (LVN) – a registered nurse helps make certain you have the resources at home to make a full recovery
Pharmacist – You may have new medications as a result of your hospital stay, so it’s important to understand why the medications were prescribed, how to take them and for how long, and how they may interact with current prescriptions
You – when you leave the hospital, there is a lot of new information and it can be confusing
The team meets with the patients for (up to three) 30-minute consultations, three to seven days after being released from the hospital.

Home Visits
PMA Home Visits program was developed to assist hospital-discharged patients with complex and/or chronic conditions to receive the best care possible. Our Providers work directly with the patient and their assigned care team during their recovery period.
PMA providers will come to the patient’s home or assisted living center for continuous care. We understand access to care may be difficult during this time; our Providers can help with referrals, prescribe and review medications, physical therapy, lab work, standard x-rays, and other home health services if applicable.
The home visits program is designed to collaborate with primary care teams and is offered free of charge without a co-pay.
eXpress Care VIRTUAL – Telehealth
Express Care Virtual is a telehealth solution designed to make health care convenient. One of our board-certified providers meets with you face-to-face through secure video technology (live) using your smartphone, tablet, or computer. Providers can diagnose and treat minor medical concerns, and prescribe medication or lab work, as needed.
$49 or less per visit – many insurance plans accepted.

Senior Wellness
A Senior Wellness Visit is one of the most important appointments you will have with your provider all year. This visit is recommended by your provider once a year and is covered for Medicare Part B individuals. At this visit, you will spend 45 minutes, one-on-one, with another provider, going over a comprehensive review and assessment of your health. Early treatment and diagnosis is the key to maintaining your health. Providence’s goal is to provide quality and compassionate care for all our patients.
